Patient case files master negative microfilm, 1998-2001.

Series consists of 16mm silver halide negative microfilm of original patient case files (1998-2001) maintained by thirty-three mental health facilities. A typical file contains admission/commitment papers, various legal documents, psychiatric evaluations, patient case summaries, correspondence, medical notes/observations, and discharge related documents. Modern case files, depending on the length of an individual's association with the Office of Mental Health, may contain between fifty to several hundred documents.

Patient case files prior to 1899 were maintained in folio sized registers that usually contained two pages of summary information for each patient. Beginning in 1899, patient case records were maintained in individual folders.
